can any one give me a rough thought of how does a claim processing business model works in healthcare/insurance companies……….

Yes. There is a book of health guidlines used by the claims people. If anything is borderline on getting coverage you can hope you get the right claims person because some are simpler than others in their choice making. Appeal each negative choice. About 50% of those appeals result in a reversal.
